Playground Permission

I give permission or my child to go on the playground and any other Community Church property. I will not hold the church or any Community Kids volunteers responsible for any accident or injury my child may incur. In the event that emergency medical attention is needed, I give my permission that it be administered by any/all Community Kids volunteers working with my child as well as any other medical personnel that may be called upon. I authorize the above mentioned persons to consent to medical treatment of injuries when I cannot be contacted for consent. If I leave the premises, I will leave emergency contact information.

Media Permission

We at Community Church would like to share photography and/or video recorded at our various ministry activities, events and programs in church-produced materials, including, but not limited to brochures, newsletters and other printed materials, on the church website and church social media accounts. Accordingly, participation in church activities implies permission for publication of any media recorded, unless a Media Opt-Out form is on file with the church office.

However, some persons may choose not to have their photos or photos of their children used and we respect their wishes. Therefore, we are providing this Media Opt-Out opportunity for families who prefer NOT to have photographs or video of themselves used by the church.
